Many insurance plans do cover services that are performed in our office, including chiropractic and massage therapy.

In order to help more patients utilize their 'IN NETWORK DISCOUNT", we are contracted with most of the insurance carriers, including the followin:

The only way that we will know if chiropractic services are covered, is if we have the proper information to verify coverage for you. If you supply us with that information we will be more than happy to verify coverage for you before you come in to see us. We will gladly file your insurance claims for you too.

We offer courtesy benefit verification if you wish.  If you would like to know how plan covers our service, please have your insurance ID Card handy when  you contact us, so that we can access your coverage and eligibility through a courtesy phone call to the carrier.

We will promptly notify you of your coverage.

DISCOUNT PLANS AVAILABLE:

We participate in a program through "PCD" or PREFERRED CHIROPRACTIC DOCTOR PROGRAM.

This is an annual membership, either individual or family, whereas the office fees are discounted for PCD members only by at least 25 percent.  This plan may be honored by other participating providers you can visit.  Please call for more details.

Information needed for Health Insurance Verification:

  1. Patient’s full name
  2. Policy holder’s name
  3. Patient’s date of birth
  4. Patient’s zip code

Auto Accident Cases:

It is very important to receive a neuro-musculo-skeletal evaluation following an accident or injury. Chiropractors are neuro-musculo-skeletal experts. Early detection through a thorough evaluation are key factors in determining if you were injured and the severity of your injuries.

We do accept accident / injury cases. We may agree to wait for payment in your case, only if we can verify your accident claim, and the claims adjuster agrees to honor a lien for payment upon settlement at the end of your treatment. Once you pick up your Police report please bring it in so that the doctor can see exactly how the accident occurred. We also need your auto insurance policy number, claim number, and your claims adjuster’s name and contact information. If you have retained an attorney, please also inform us, so that we may best help your counsel when they need your treatment records.

Work Injury:

Prior authorization from your employer or a medical referral is usually required for all work injury treatment. Check with your employer first, and ask if it is permitted for you to seek chiropractic evaluation for your work related injury. Our office staff will assist you in completing the work injury paperwork for authorization.

Massage Therapy:

All chiropractic patients who receive massage therapy will need to pay their portion for the therapy as it is rendered, unless other arrangements were made in writing prior to the therapy session. This is necessary so that the therapist can be paid for their work each week. If your insurance policy covers massage therapy, you will be reimbursed once the insurance carrier pays you directly or sends payment to us. We make every effort to attempt to verify coverage on massage therapy when the doctor recommends this type of therapy.

Payment plans are always an option, if you cannot afford to pay your full portion as you receive treatment. All payment plans must be set up with our insurance department in writing.

We will work hard to help you achieve your goals and to lessen your worries on the financial end, by working closely with your insurance representative or attorney, supplying treatment documentation when requested and by submitting claims in a timely manner.

Appointment Cancellation Policy:

We ask that if you cancel your appointments for any reason, even if a last minute emergency arises, that you phone our office immediately. The time which was allotted for you with either the doctor or the therapist can be given to another patient if needed. Without 24-hour cancellation notice on massage therapy, you will be responsible for payment in full to the therapist for the time set aside for you, unless you had an unavoidable emergency situation.

Cost will vary depending on the types of treatment performed on each visit. Please feel free to inquire about fees before beginning treatment or at any time during your care.
